How to Request to Reprint Lost or Damaged Health Card in Qatar
Individuals in Qatar, whether citizens or residents, can avail of the Qatar Health Card in order to enjoy special benefits when it comes to health care services in the country. Special privileges include certain subsidies for consultations or non-emergency treatments. Prescriptions can also be filled at subsidized prices for anyone who holds a valid Qatar health card provided the prescriptions are filled at any pharmacy that is run by the government.
In case anyone loses his or her health card, or their health cards are damaged somehow, they can apply to request for a reprint of the lost or damaged health card from the Hamad Medical Corporation (HMC).
You simply need to fill out the form online and provide the necessary information that is required. Input your Qatari ID number next. Then, when you get to the “Card Information” page, choose “Renew (Update Expiry Date)” and hit “Next”. Choose the “Number of Required Years” for your card renewal. In the “Application Form” page, you must enter your telephone number accurately.
Should you wish to receive your receipt voucher by email, select “Yes” and enter your correct email address. If you select “Yes” to receive notifications via SMS, input your mobile phone number as well.
Afterward, make sure that you click “Next”. This will take you to the “Payment Details” page, where you can select “Pay” and submit the necessary fees for the application.
If, by any chance, you wish to complete the process offline, you can pay a visit to any of the conveniently located HMC facilities or health centers that are affiliated with the Primary Health Care Corporation (PHCC).
Once there, you will need to submit a copy of your passport or your valid ID, as well as a 4cm x 3cm photograph (this is standard passport size) for Qatari adults. Qatari children should submit their ID number, while any adult GCC National should submit a copy of their ID with the Qatar personal identity number. In case of a damaged card, they should also submit their old health card.
Children who are GCC Nationals need a valid copy of their ID along with their Qatar personal identity number as well. The same procedure applies for damaged old health cards.
Meanwhile, any adult resident needs to provide their QID along with their valid residence permit. Children need to submit the same. Domestic staff, on the other hand, need to submit their QID with valid residence permit, as well as the ID health card and original QID of their sponsor.
When you need to claim your reissued health cards, keep in mind that you can only do so at the PHCC centers during morning working hours. HMC facilities can accommodate your request during both morning and evening work hours. You will either be able to receive your new health card right away or after a few days once notified that it is ready for pick-up.
