How to Issue Health Card in Qatar
The citizens of Qatar as well as residents may easily apply for a health card in order to access the convenient services that are provided by the hospitals and public healthcare facilities in the State. Simply pay a visit to the nearest Primary Health Care Corporation (PHCC) center in your location. You will have to fill out the necessary forms. Provide all of the appropriate documents and personal data. Afterward, a PHCC health file number will be issued to you.
Once prompted, you will have to present your passport copy, your ID, and a passport-size photo (4cm X 3cm) for Qatari adults. Children need to provide their birth certificate with their Qatar personal identity number or passport. They will also be required to submit a passport-size photograph. For first-time issue, the vaccination card will also be required.
For GCC National Adult Residents, you will need to submit a copy of passport. You also have the option to submit your ID with the Qatar personal identity number. Submit a passport-size photograph. If you will be renewing your old health card, bring along your old health card as well.
GCC National Child Residents need to submit their birth certificate or passport with Qatar personal identity number. A passport-size photo will be required upon application. For children who are born in Qatar and for first time issue, the vaccination card is required.
Adult Residents will need to provide a valid QID. Alternatively, you may submit a passport with valid residence permit as well as a passport-size photo. Children need to submit a passport with valid residence permit, as well as a passport-size photo and their vaccination card if they had their vaccinations in the country.
Domestic Staff have to submit a valid passport with valid residence permit along with a passport-size photo and the original QID of their sponsor. For everyone, a copy of the utility bill is required in order to show proof of residence. You may use your phone bill, your water bill, or your electricity bill for proof.
Bear in mind that your health card will contain information about your ID number, your nationality, and your photo and the date of expiration of the card. All residents of Qatar may access this health card. Once you submit your application to the nearest health center, you will either be able to receive the card right away or you will be asked to come back in a few days to collect your issued health card.
The health card will be able to help you with subsidies for consultations or non-emergency treatments. Your prescriptions will also be filled at a government-run pharmacy with lower rates provided you pay using your credit card and not in cash. Once your health card expires, you may renew it at the HMC hospitals near you.
